   ·  Click on link above - select SAVE
   ·  Save in My Documents
   ·  Double-click on the header (page 1) to fill-in
   ·  Will auto-complete header on all pages
   ·  Type a capital X to complete applicable items in right-hand column
   ·  When finished, print or attach to e-mail as per Principal's instructions
    
    

2. Adobe Acrobat PDF  (revised June 2001)
   
   ·  Click on link above - select SAVE
   ·  Save in My Documents
   ·  Click on printer icon to print hardcopy
   ·  Complete manually in ink - no pencil
   ·  When finished, submit as per Principal's instructions
